Well done! Very complete and well organized.

And as a side note, speaking as a Ski Patroller with Mountain Travel and Rescue certification I REALLY like the "Emergency Response - Patient Report" form for use at the scene. It is very self-explanatory and the organization is excellent with the priorities of patient injury/illness evaluation in the perfect order. The tear-off form for sending with mesengers is also quite well done.

All in all it's the best form of its type I've seen to date.
As the Chapter Outings Chair I ask that all outings leaders in my chapter carry this form.

Also I have recommended it to the National Ski Patrol Far West Division's Mountain Travel and Rescue instructors for off-resort travel and rescue.
